If you are already a member of Congregation Mogen David, we ask you to please complete a Membership Form so that we have your current information. This will help us to better serve you and your needs.

**ASSOCIATE MEMBERS: Please note this membership category is only for those individuals or families who are members of another shul, but would, nevertheless, like to be associated with ours. Associate Members are welcome to attend any event, Shabbaton, and Holiday celebrations but, are not entitled to High Holiday seats, unless purchased separately. Associate Membership is $750 per family, $400 for single membership. Associate members are not entitled to vote at the Membership meeting.
